SWITCH KIT

When opening an account, two forms of identification are needed for all new account holders. One form should contain a picture and signature with expiration date.

Acceptable identification for the first form:

  • Valid Driver’s License
  • Valid Wisconsin State ID
  • Valid Military Identification
  • Valid Passport

The second form should either be one of the above or any of the following: 

  • Alien Registration Card – must have picture & signature
  • Employment Identification Card – must have picture & signature
  • Wisconsin School ID Card – with picture & signature
  • Any government issued ID containing photo, signature, issue and expiration date.
  • Social Security card
  • Major Credit or Debit Card

Also, if you are not in the phone book please provide a current utility or other bill to verify your current address.
