OPENING NEW ACCOUNTS? SWITCHING IS EASY

When opening an account, two forms of identification are needed for all new account holders. One form should contain a picture and signature with expiration date.

Acceptable identification for the first form:

  • Valid Driver’s License or Driver’s License receipt*
  • Valid Wisconsin State ID over 60 days old or Wisconsin State ID receipt*. Out of State ID is not accepted
  • Valid Military Identification –record issue date, place and expiration
  • Valid Passport –record issue date, place and expiration

*Receipts are valid for 45 days from date of issuance.


The second form should either be one of the above or any of the following:

  • Alien Registration Card – must have picture & signature
  • Employment Identification Card – must have picture & signature –record issue date, place and expiration
  • Wisconsin School ID Card – with picture & signature–record issue date, place and expiration
  • Any government issued ID containing photo, signature, issue and expiration date.
  • Social Security card
  • Major Credit or Debit Card-document issuer and expiration date

Also, if you are not in the phone book please provide a current utility or other bill to verify your current address.
